'Winter Pressures' by Accord Group

We operate a joint health-housing case management initiative to reduce high numbers of A&E admissions from Bennett House. We do this by upskilling our care staff and proactively managing resident’s health needs. Bennett House is an Accord Group care home for 45 older people in the West Midlands, and we have 26 places for people with dementia. Find out more.

Supporting People off the Streets (SPOTS)' by Axiom

We aimed to prevent first night rough sleepers from becoming trapped, with all the associated risks to their health, and the consequential impact on health and social care services. Our scheme focussed on improving outcomes for first night rough sleepers and reducing the number who spend more than one night sleeping out. Find out more.

'Wellbeing service' by Walsall Housing Group

Walsall Housing Group Wellbeing Service supports vulnerable people to maintain independence and reduces dependence on more intensive forms of care and support. It provides a cost effective mechanism for addressing the need for support to those who do not meet the eligibility criteria for social care services in order to prevent or delay the need for higher cost services. Find out more.
